The HCS program provides individualized services and supports to persons with intellectual disability and developmental
disabilities who are living with their family, in their own home or in other community settings, such as small group homes.

Residential Support
Residential Support
This residential option provides HCS Residential Group Homes that are 3 or 4 bed group homes that are available through the Home and Community Based Services Medicaid waiver program, which is certified by the state. Residential Group Homes provide a safe and comfortable living for up to 4 residents in the Houston area. Provided with our group homes are 24/7 support services that assist residents with the activities of daily living (ADL)
Supported Home Living
Supported Home Living
This residential option is supported by trained staff that coordinates and provides services for the individual who chooses to live in their own home or in their family's home. Supported Home Living services include meal planning and preparation, transportation, assistance with housekeeping and many more services and supports.
Foster Companion Care
Foster Companion Care
This residential option provides a home and support services for the individual by a foster care provider, the individual can choose to live in a foster home with a family member or other individual as the foster care provider.
Jaru HCS program contracts with the foster care providers to assist with the daily living skills, personal care, transportation and other activities that allow the individual to live as independently as possible in a safe and supported environment.
Respite Care
Respite Care
In and out of Home Respite. Short term care provided for primary caregiver to help with vacation time, out of town business, or simply to give the individual free time to spend with peers.
Home Modification
Home Modification
Assistance is provided in the acquisition of mobility aids, environmental controls, medically necessary supplies, and communication aids.
Day Habilitation
Day Habilitation
This option provides assistance with day activities, the individual will participate in recreational activities. Day Hab is designed to assist the individual to acquire, retain, and improve self-help, socialization, daily living skills and set personal goals.

Everyone who is enrolled in HCS has a service coordinator who:
Coordinates the development and implementation
of the Individual Service Plan (ISP). Individual Plan of Care, integrated services delivered and documents the consumer's progress. Coordinates various aspects of services delivered under the HCS Program and through other sources. Reviews issues related to rights, safety and health.
